After.Care provides a simple way to gather data to improve the life of patients after care.
After.Care presents a series of yes or no questions. Each question is designed to measure physical and mental health of the participant. At the completion of the survey, the results are sent to the users doctor or hospital.
This survey is based on the RAND Medical Outcomes Study SF-36 survey.
These questions have been altered for the purpose of this application.
